COMMUNITY SUPPORT WORKER
Mental Health Resources, Inc. is looking for a responsible adult to work with mentally ill (SPMI) adults in Platte, Clay and Ray counties. Duties include assisting clients with medication, accompanying clients to various appointments and helping clients stay out of the hospital. Work hours are 8-5, M-F.
Mental Health Resources, Inc. is a contract provider for Beacon Mental Health.
Responsibilities include:
Develop timely and goal/client oriented progress notes
Develop annual treatment plan that is strength based, reflecting client’s goals and desired outcomes
Complete other required documentation (CIP’s, CSW/Client agreement, and progress notes) in a timely manner
Maintains collateral contacts which are deemed appropriate by treatment plan or crisis intervention
Promotes client participation in the community by assisting clients to cultivate community resources in order to maintain daily living skills
Assists and supports clients in making and carrying out health care needs decisions as indicated in treatment plan and/or documented in progress notes
Include client, client guardian, secondary consumer, RCF staff, etc. in annual treatment planning, quarterly reviews and critical intervention plans
Follow procedures for crisis intervention and will seek team/QMHP for consultation/resolution
Requirements:
BA/BS in Psychology, Social Work, or related field
Valid Driver’s License
